Job summary

Conemaugh Memorial Medical Center, part of Lifepoint Health, invites an RN to join the Transitional Care Unit (TCU). This 30-bed unit provides short-term rehabilitation with daily nursing care and rehab therapies.

Typical shifts are 12 hours, with rotating weekends and holidays, and a supportive team environment focused on safe transitions home. The RN will assess resident needs, manage orders, supervise care, and participate in care planning and patient education, contributing to high-quality

Qualifications

• Current state RN license is required.
• Bachelor's degree from an accredited nursing school.
• Basic Life Support certification required within 30 days of hire.
• CPR Certification.
• Two years of nursing experience preferred.
• One year of long-term care experience preferred.

Responsibilities

• Assess needs of residents and establish priorities for resident care based on medical and nursing diagnoses.
• Reassess and evaluate changes in resident's condition and document changes.
• Assist with the completion of the Minimum Data Set (MDS).
• Assess learning needs of residents and/or families and provide teaching per care plan.
• Document pertinent data in the resident's medical record.
• Initiate and transcribe physicians' orders; administer medications per policies.
• Monitor IV therapy and participate in policy/procedure reviews and updates.

Education

Bachelor's degree in Nursing

Registered Nurse license

Basic Life Support

CPR Certification
