Licensed Practical Nurse (LPN) – Transitional Care Unit

Job Type: Full‑time | Variable 8‑ & 12‑hour shifts, every other weekend and holiday rotation. No on‑call. Up to $15,000 sign‑on bonus. Your experience matters.

About Our Team

TCU is a 30‑bed short‑term skilled hospital‑based rehabilitation unit at MMC. Patients receive daily nursing care and a minimum of two hours per day of PT, OT, & SLP. TCU has 24 patient rooms – 18 private and 6 semi‑private. Patients stay 10–12 days to recover, build strength, and go home. They come for post‑op care, wound care, teaching/training for new diagnoses, IV therapy, pain, and chronic illness management.

Census up to 24: 7P = 2‑3 licensed nurses – RN/LPN combination; 2 CNAs. A: 3‑4 licensed nurses – generally 2‑3 RNs and 1‑2 LPNs, plus 2‑3 CNAs.

How You’ll Contribute

• Follow the plan of care established by the professional nurse and collaborate with members of the health‑care team.
• Recognize, report, and document changes in the patient’s condition.
• Observe and document the patient’s physiological condition (appearance, vital signs, level of consciousness, behavior changes) and report to RN.
• Demonstrate skill and knowledge in carrying out and documenting all nursing interventions ordered by a physician (e.g., medication administration, treatments, assistance with ADL).
• Document the patient’s response to interventions carried out.
• Consistently provide and document reinforcement of patient teaching.
