Northwestern Medicine Palos Hospital is hiring a Registered Nurse for Transitional Care on the night shift. This role focuses on caring for patients boarding in the Emergency Department awaiting inpatient beds, with telemetry and Med/Surg experience highly valued.

The RN provides nursing care, monitors patients, administers medications, supports discharge planning, and collaborates with inpatient teams to ensure a safe and smooth transition of care. A BSN and IL RN license are required.

Qualifications

• BSN degree required and IL RN license.
• BLS required; ACLS/PALS/TNCC within 1 year for ER/Pedi/Non-Invasive Cardiology as applicable.
• ADN acceptable with commitment to obtain BSN within 3 years.

Responsabilités

• Assess and record patient status on admission and at point of care.
• Organize health data to create an accurate, accessible plan of care.
• Identify problems needing immediate attention.
• Document diagnoses and plan of care.
• Plan discharge at admission and coordinate with patient/family and team.
• Provide patient and family education based on evidence-based practice.
• Collaborate with inpatient teams and support discharge processes.
• Prioritize evidence-based nursing interventions and reassess progress.
