Job summary

Responsibilities include guiding patients through diagnosis to follow-up, creating individualized care plans, and ensuring timely discharge planning and utilization management.

Qualifications

• Bachelor's degree in Healthcare Administration required.
• Bachelor's degree in Nursing preferred.
• 3 years minimum in utilization review, discharge planning, case management or disease management preferred.
• Nursing experience in home or ambulatory settings with high-risk patients beneficial.
• 2+ years of clinical nursing experience preferred.
• IDFPR RN license required in Illinois.
• Strong interpersonal and communication skills.
• Experience with EMR and Microsoft Office.
• Knowledge of InterQual/MCG criteria preferred.

Responsibilities

• Guides high-risk patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care.
• Establishes an individualized plan of care for assigned patients using evidence-based guidelines and patient goals.
• Partners with the healthcare team to ensure timely clinical decision-making and discharge planning.
• Performs daily coordination between multiple departments and care teams to assure patient safety and smooth transitions.
• Acts as advisor/educator, providing clinical education and resources to patients and families.
• Facilitates appointments for consultations and support services within protocols.
• Completes Utilization Management using Milliman Care Guidelines and monitors LOS and resource use.
