Job summary
Endeavor Health in Arlington Heights, IL, seeks a RN Transitional Care Navigator to manage case, care coordination and utilization across care levels. You will guide high‑risk patients, create individualized plans, and coordinate with a multidisciplinary team to promote safe transitions and cost‑effective outcomes.
This full‑time role requires IL RN licensure, 3+ years in utilization review or case management, and strong collaboration, EMR and data skills.
Qualifications
- RN license in Illinois (IDFPR) required or eligible.
- 3+ years in utilization review, discharge planning, case management, or disease management.
- Experience with home/ambulatory services for high-risk patients preferred.
- Proven ability to coordinate care across settings and teams.
Responsibilities
- Guide high‑risk patients through diagnosis, treatment, and follow‑up.
- Develop individualized care plans aligned with patient goals.
- Coordinate discharge planning and multidisciplinary care.
- Monitor utilization and cost of care; promote safe transitions.
