The role requires RN experience and strong utilization management skills, with 2–3 days onsite and 2 days remote. Travel to patient homes may be needed; weekend/holiday coverage applied by rotation.

Qualifications

• Three years in utilization review, discharge planning, or case management.
• RN experience in home/ambulatory settings favored.
• Familiar with InterQual/MCG criteria is preferred.

Responsibilities

• Guides high‑risk patients through the health system from diagnosis to follow‑up care.
• Establishes individualized care plans using evidence‑based guidelines.
• Partners with the healthcare team to ensure timely decision-making and discharge planning.
