Job summary

FMOLHS in Louisiana seeks an RN Transfer Center professional to coordinate safe transfers for emergent or urgent medical needs. Under the house manager, you will ensure timely communication with providers, clinical staff, and transport teams, while guiding decisions through EMTALA guidelines.

The role requires strong pediatric and adult clinical knowledge, EMR proficiency, and the ability to triage by phone, document transfers accurately, and build collaborative relationships across FMOLHS

Qualifications

• Three years in general or specialty nursing practice.
• Louisiana RN licensure required.
• Strong pediatric and adult clinical skills and ability to assess and triage via phone.

Responsibilities

• Coordinates and manages patient transfer requests.
• Expedites and coordinates internal and external transfers with physicians and departments; maintains awareness of FMOLHS services.
• Document transfers in the EMR; provide medical records and transfer information to stakeholders.
• Ensure EMTALA compliance and patient throughput.
• Provide superior customer service and collaborate with hospital staff and external providers.
• Perform patient assessment and triage to determine level of care and appropriate facility.
