Qualifications

• Three years of general or specialty nursing practice.
• Licensure as a Registered Professional Nurse in Louisiana.
• Strong pediatric and adult clinical skills and rapid phone assessment.

Responsibilities

• Coordinates and manages patient transfer requests.
• Expedites and coordinates all internal and external transfers between FMOLHS facilities and receiving sites.
• Maintains Transfer Center documentation and compliance in the EMR and department databases.
• Provides customer service and escalates issues per departmental protocols.
• Performs patient assessment and triage for emergent or urgent transfers, including trauma, stroke, and STEMI considerations.
