Job summary

Bon Secours is seeking an RN Transfer Center Specialist to coordinate patient transfers, acceptance, and bed placement. You will work with referring facilities, physicians, and hospital staff to ensure timely, safe care and accurate patient documentation in an environment focused on quality and continuity.

The role requires active RN licensure, 1 year of acute care experience, and on-site operations across all shifts with an every-other-weekend obligation.

Qualifications

• Active RN license in the applicable state is required.
• 1 year of acute care experience is required.
• ADN/diploma is required; BSN preferred.

Responsibilities

• Coordinate transfer workflows to identify appropriate level of care and bed assignment.
• Collaborate with referring facilities, physicians, and hospital staff to expedite acceptance and care plan.
• Maintain accurate patient information and protect PHI; ensure timely data entry and documentation.
