Job summary

Advocate Health is seeking a Registered Nurse to coordinate patient transfers from the 70518 Atrium Health Mobile Medicine – Care Coordination Center. The role requires on-site work, 36 hours per week (7p–7a), and strong coordination with physicians, transport staff, and care teams to ensure safe patient placement.

Candidates should have ADN and RN licensure, with a BSN preferred and experience in triage, EMR use, and interprofessional collaboration.

Qualifications

• ADN required; RN licensure in relevant state.
• RN with minimum 3 years nursing experience.
• BSN preferred; CAVRN certification beneficial.

Responsibilities

• Coordinate safe, timely patient transfers; work with physicians, transport staff, and providers.
• Perform triage to determine care level and subspecialty needs for transfers.
• Serve as liaison between referring and receiving facilities; document transfer requests.
• Collaborate with interdisciplinary teams to ensure patient-centered care and escalation when needed.
• Document transfer activities in EMR and transfer-center software; generate monthly reports.
