Job summary

CHRISTUS Health's Transfer Center RN coordinates patient transfers into the CHRISTUS Health System, collecting medical and demographic information and communicating with referring and receiving physicians, ED and inpatient staff to ensure safe, efficient transfers.

This full‑time position requires an Associate's Degree in Nursing, RN license, BLS, and strong customer service, problem solving, and teamwork skills; schedule is 11 AM to 11 PM.

Qualifications

• Associate's Degree in Nursing required.
• RN license in state of employment or compact.
• BLS required.
• Excellent customer service and communication skills.

Responsibilities

• Exhibits excellent customer service with physicians, case managers, nurses, administrators and colleagues.
• Maintains professionalism and patient-centered attitude.
• Anticipates and prioritizes transfer issues and escalates barriers per hospital policy.
• Collaborates with ED, House Supervisors and physicians to ensure timely transfers.
• Maintains knowledge of bed availability, capacity and patient flow; updates service continuity.
