Job description

Overview

The Transfer Center’s mission is to ensure safe, timely, and appropriate patient care by connecting providers and coordinating resources. The Clinical Access Coordinator communicates with referring providers, matches patients to the most appropriate receiving provider based on medical need, and advocates for safe, efficient care through accurate triage and coordination of services.

Responsibilities

• Fielding over 100 calls per shift (busier in the day than overnight)
• Serves as a centralized intake and triage hub for pediatric and adult patients, coordinating transfers, bed placement recommendations, and prioritization of critically ill or injured patients to ensure timely access to care.
• Provides real-time clinical triage and emergency support using the nursing process, critical thinking, and telecommunication tools to manage acute changes in patient condition and escalate care as needed.
• Facilitates timely provider-to-provider consultations, handoffs, and interdisciplinary communication to support safe, appropriate patient placement and continuity of care.
• Coordinates care activities across patient care units and hospital departments, anticipating and resolving barriers to patient flow and treatment.
• Collects clinical data and delivers virtual nursing support to optimize patient outcomes, support evidence-based practice, and promote safe, efficient, patient-centered care across acute and critical care settings.

Requirements

• Bachelor's Degree in Nursing (would accept someone with an associate's degree in nursing who is currently enrolled in a Bachelor’s program)
• MA RN License
• Emergency department clinical experience

Schedule/Shift

• 24 hours per week (Two 12 hour shifts)
• Flexibility to pick up additional shifts
• Every third weekend and every third holiday
