Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Minimum 3 years' related clinical experience in an acute care setting.
• RN license in the states where facilities are located; multi-state if needed.
• Knowledge of hospital operations and patient care practices.
• Strong problem solving, analytical and communication skills.
• Ability to establish collaborative relationships and work across teams.

Responsibilities

• Performs transfer center functions with centralized patient placement.
• Coordinates transfer of emergent, urgent, and elective patients to final facilities.
• Facilitates patient throughput with focus on quality and efficiency.
• Promotes teamwork and effective communication between facilities.
• Ensures timely bed placement and accurate initial plan of care.
• Maintains data in patient records and center workflow systems.
• Navigates technology platforms and documents appropriately.
• Uses critical thinking to assess situations and patient flow.
• Triages and communicates clinical presentations to accepting physicians.
• Advises on patient growth, development, and care needs across ages.
