You will collect clinical data and match patient needs to bed capacity, coordinating with referring facilities, physicians, and hospital staff to optimize throughput and safety.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Minimum 3 years of related clinical experience in an acute care setting.
• RN license in the states where the facilities are located; multi-state license may be required.
• Knowledge of hospital and clinical operations and patient care practices.
• Ability to communicate effectively and work independently while coordinating with multiple departments.

Responsibilities

• Performs all transfer center functions in a timely, patient-centered manner using a computer database and facility profiles.
• Coordinates transfer of emergent, urgent, and elective patients to a final destination facility from all referral sources.
• Facilitates patient throughput and follows up with all participants of the transfer.
• Promotes teamwork and professional collaboration with referring facilities, physicians, and hospital staff.
• Ensures timely bed placement and accurate initial care plans for patients.
