Job summary

Job Juncture seeks a seasoned RN leader to supervise day-to-day clinical and office operations in a home health setting. You will ensure care meets standards, coordinate interdisciplinary care, and oversee care plans and regulatory compliance.

You will guide clinicians on emergency protocols, mentor staff, and drive QA activities while supporting accreditation readiness and audits. Travel to multiple locations may be required.

Qualifications

  • Graduate from an approved school of professional nursing and licensed as an RN in the state of operation.
  • 3–5 years experience as an RN.
  • Experience managing RN services in the home, community, or clinic preferred.
  • Must possess and maintain CPR certification while employed in a clinical role.
  • Must be a licensed driver with a vehicle that is insured and travel to all locations.

Responsibilities

  • Supervise day-to-day clinical and office operations.
  • Ensure services meet professional standards and regulatory requirements.
  • Coordinate patient referrals and care with the interdisciplinary team.
  • Oversee development and updating of individualized care plans.
  • Evaluate quality of clinical care and assess patient needs.
  • Collaborate with clinicians on transfers, discharges, or changes in condition.
  • Manage OASIS documentation, edits, discharge processes, and related data.
  • Monitor KPIs, workflows, and escalations.
  • Provide training, supervision, coaching, and mentoring to clinicians.
  • Act as QA/PI owner and participate in QA activities and corrective actions.
  • Support accreditation readiness and QA audits.
  • Serve as backup for other roles and lead in absence of Territory Administrator.

Job description

Supervise day-to-day clinical and office operations; remain available to staff and patients during business hours.

Ensure all services meet professional standards, agency policies/procedures, and state/federal regulatory requirements.

Coordinate patient referrals and care with the interdisciplinary team.

Oversee the development, implementation, and updating of individualized plans of care.

Evaluate the quality of clinical care and continually assess patient needs.

Collaborate with clinicians and physicians on patient transfers, discharges, or changes in condition.

Manage OASIS documentation, edits, discharge processes, and related data.

Monitor key performance indicators (KPIs), workflows, and escalations.

Guide clinicians on emergency care protocols.

Provide training, supervision, coaching, and mentoring to clinicians (including contract staff); assign patients/staff based on competence.

Act as QAPI (Quality Assessment and Performance Improvement) owner; participate in QA activities, investigate incidents, analyze trends, and implement corrective actions.

Support accreditation readiness, QA audits, HHCAHPS surveys, and emergency preparedness planning.

Serve as backup for other roles (e.g., Patient Care Manager) and act as leader in the absence of the Territory Administrator.

Manage operational aspects in collaboration with the team to support exceptional home health care.

  • 3-5 years experience as a RN.
  • Experience managing RN services in the home, community, or clinic is preferred

Required Certifications and Licensures:

  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
