Nurses at Home is seeking an RN Clinical Case Manager to join our small, clinician‑led home health team in Waterbury, CT. You will manage patient plans and coordinate care on the road with a supportive, owner‑led culture.

The ideal candidate holds an RN license, has 2+ years in home health (OASIS proficiency preferred), and English language skills. A Bachelor's degree is preferred. We offer top industry rates and growth opportunities.

Responsibilities

• Manage clinical caseload in home health settings.
