Job summary

WellSky in the United States is seeking an experienced RN to perform telephonic patient evaluations across Pacific time zone hours. The role includes assessing level of care, documenting plans, and coordinating with internal and external stakeholders.

Qualifications include active RN licensure, 2–4 years of related experience and a bachelor’s degree or equivalent. Full-time role with strong benefits and opportunities to contribute to program improvements.

Qualifications

• Active RN licensure is required.
• 2–4 years related work experience.
• Experience in emergency departments is preferred.

Responsibilities

• Conduct telephonic assessments to determine level of care for patients per program protocols.
• Schedule and perform telephonic visits and document assessments with follow-up plans.
• Review patient assessments to identify trends and opportunities for improvement.
• Collaborate with internal and external stakeholders to coordinate patient visits.
• Assist with reporting on program progress and KPIs as required.
• Participate in planning meetings to develop protocols and policies.
• Perform other duties as assigned.
