Job summary

WellSky is seeking an RN with 2+ years of related experience to conduct telephonic patient evaluations for various payer programs. The role supports Pacific time zone hours and requires scheduling, documenting assessments, and following up on care plans.

You will review patient data, identify improvement opportunities, and collaborate with internal and external stakeholders to ensure timely visits and accurate reporting on program progress. A strong emphasis on security and compliance is noted.

Qualifications

• Must have RN licensure and 2+ years of related work experience.
• Experience in emergency departments or acute/post-acute settings preferred.
• Bachelor's degree or equivalent experience required.
• Experience coordinating care and communicating with multiple stakeholders is beneficial.

Responsibilities

• Conduct telephonic assessments to determine level of care per program protocols.
• Schedule and perform telephonic visits; document assessments and follow-up plans.
• Review assessments to identify trends and opportunities for improvement.
• Collaborate with stakeholders to ensure timely and accurate patient visits.
