Qualifications

• Active RN licensure.
• 2–4 years related work experience.
• Experience in emergency departments preferred.

Responsibilities

• Conduct telephonic assessments to determine level of care following program protocols.
• Schedule and perform telephonic visits, document assessments, and communicate follow-up plans within established timelines.
• Review and analyze patient assessments to identify trends and opportunities for improvement.
• Collaborate with internal and external stakeholders to ensure timely and accurate patient visits and address scheduling needs.
• Assist with reporting on program progress, outcomes, and other key performance indicators as required.
