Job summary

WellSky is seeking a registered nurse to perform telephonic patient evaluations across multiple payer programs. The role requires coordinating telephonic visits, documenting assessments, and communicating follow-up plans in line with program protocols.

The ideal candidate holds an active RN license with 2–4 years of healthcare experience, including time in emergency departments, and is comfortable working Pacific Time hours.

Qualifications

• Active RN licensure is required.
• 2-4 years related work experience in healthcare.
• Experience in emergency departments is preferred.

Responsibilities

• Conduct telephonic assessments to determine level of care per program protocols.
• Schedule and perform telephonic visits, document assessments, and communicate follow-up plans within timelines.
• Review patient assessments to identify trends and opportunities for improvement.
• Collaborate with internal/external stakeholders to ensure timely patient visits and scheduling.
• Assist with reporting on program progress, outcomes, and KPIs as required.
