Job summary

WellSky is seeking a registered nurse to perform telephonic patient evaluations and assessments across its payer programs. The role supports Pacific Time zone hours and requires collaboration with both internal teams and external partners to ensure timely visits and accurate documentation.

The ideal candidate holds an active RN license, 2–4 years of related experience (preferably in emergency departments), and a Bachelor’s degree or equivalent.

Qualifications

• 2–4 years related work experience required.
• Active RN licensure required.
• At least 2 years’ experience in emergency departments.

Responsibilities

• Conduct telephonic assessments to determine level of care for patients in adherence with program protocols.
• Schedule and perform telephonic visits, document assessments, and communicate follow-up plans within established timelines.
• Review and analyze patient assessments to identify trends, opportunities, and areas for improvement.
• Collaborate with internal and external stakeholders to ensure timely and accurate patient visits and address scheduling needs.
• Assist with reporting on program progress, outcomes, and other key performance indicators as required.
