Job summary

CommonSpirit Health is seeking an RN Telephone Triage professional to join the Call Center in Longmont, CO. You will manage calls, perform health assessments, and provide general health information along with physician or community service referrals as needed.

The role requires an RN with strong telephone triage skills and the ability to work efficiently in a multi-task environment. You will document interactions and educate patients and families while upholding patient rights.

Qualifications

• Associate's Degree in Nursing or by endorsement.
• Three (3) years recent hospital, home health or clinic experience.
• RN License; Current state nursing licenses or valid RN license from a participating state in the National Licensure Compact.
• Current Basic Life Support (BLS) certification issued by the American Heart Association.

Responsibilities

• Answers incoming calls for all product lines and provides thorough and accurate telephone triage.
• Assesses patient's physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records & clinical data.
• Evaluates patient's response and intervenes to ensure optimal patient outcome.
• Documents all calls according to call center guidelines.
• Promotes, advocates, and collaborates to protect the health and safety and rights of each patient.
• Provides patient/family education through assessment of patient/family learning readiness, needs, and ability.
• Recognizes the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patient's preferences, values, and needs.
