Job summary

Jobtailor is seeking a dedicated Registered Nurse to plan, organize, and direct home health services in Massachusetts. The role requires strong clinical assessment, care planning, and collaboration with families to tailor in-home care.

Responsibilities include documenting assessments, initiating plans of care, and supervising home health aides. Eligibility includes active MA RN license and BLS; OASIS/EPIC experience is a plus. This is a full-time, on-site position with competitive compensation.

Qualifications

Job description

• Plan, organize, and direct home care services
• Build services from community resources to meet patient and family needs in their homes and communities
• Complete and document initial patient and family assessments
• Initiate and revise plans of care as patient status and needs change
• Administer physician-prescribed medications and treatments
• Instruct, supervise, and evaluate home health aide care every two weeks
• Identify discharge planning needs
• Practice confidentiality according to agency and Federal HIPAA guidelines
• Complete OASIS documentation
• Complete EPIC documentation according to agency timeliness policies

Requirements

• Active valid Massachusetts RN license
• Active American Health Association BLS certification
• Associate’s Degree or BSN from an accredited school of nursing (BSN preferred)
• At least 1 year of direct care nursing experience
• Public health or home care nursing experience preferred
• OASIS and/or EPIC experience preferred
• Must be vaccinated against influenza as a condition of employment
