Job summary

Jobtailor is seeking a registered nurse to oversee patient care services in a Home Health/Hospice setting in Alexandria, LA. You will supervise staff, coordinate patient care and referrals, and ensure timely, compliant service delivery.

The role requires a nursing degree, current RN license, and 2 years in home health. Travel and field work are expected as part of routine coverage.

Qualifications

• Associate degree in nursing is required.
• 2 years of home care experience as an RN in a Home Health or Hospice environment.
• 1 year of supervisory experience preferred.
• Current, unrestricted RN license valid for the state of work.
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase and Workday.
• Knowledge of Microsoft Office.

Responsibilities

• Provide oversight of patient care services and personnel.
• Make patient and personnel assignments.
• Coordinate patient care and referrals.
• Continually assess patient needs.
• Develop, implement, and update individualized patient plans of care.
• Plan, direct, and participate in delivery of home health services.
• Coordinate services with providers, vendors, and facilities.
• Ensure timely start of patient care within regulatory guidelines.
• Process and coordinate physician orders, staff assignments, and communications.
• Review and approve plans of care and evaluate proposed clinical changes.
• Integrate orders from relevant physicians and ensure responsible-physician approval.
• Conduct regular OASIS reviews for all patients.
• Meet regularly with direct care team members for guidance and updates.
• Initiate hiring requests, interview candidates, and recommend hiring of direct care staff.
• Communicate with Branch Director or Administrator on key performance metrics.
• Remain available for field work as needed.
• Perform other assigned duties and projects.

Requirements

• Able and willing to travel within branch/office coverage area
• Dependable vehicle
• Valid driver's license
• Current auto insurance in accordance with state laws
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
