Job description

• Complete intake and annual assessment and admission visits
• Complete required documentation, including client assessment forms and multidisciplinary plan of care
• Communicate professionally with clients’ PCPs and other healthcare providers when necessary
• Review client admission binder information and educate caregivers on the required caregiver communication tool or electronic visit verification system
• Complete regular in-home visits
• Provide caregiver education and skills training
• Assess health and environmental changes
• Conduct semi-annual visits
• Review and update plans of care and corresponding goals
• Perform medication review and reconciliation
• Review caregiver communication tools
• Address environmental, psychosocial, and physical-health concerns
• Participate in multidisciplinary team meetings
• Collaborate with the team to address client needs
• Perform other duties within the scope of practice as assigned by management

Requirements

• Willingness to travel within assigned region required
• Valid driver’s license and reliable transportation required
• Licensed as an RN by Massachusetts Board of Registration in Nursing, required
• Minimum of two-year recent experience in the direct care of elders or adults with disabilities
• Fluency in both English and second language strongly preferred
• Case management, behavioral health, social services and/or community health worker experience preferred
• Strong communication, documentation and organizational skills required
• Must be solution oriented and able to multitask
• Ability to drive safely
• Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse
• Requires lifting and carrying equipment and supplies weighing up to 35 pounds
• Requires pushing and pulling equipment and supplies weighing up to 35 pounds
