Job summary

Jobtailor is seeking a Registered Nurse for home health services in Massachusetts. The role involves planning, organizing, and directing patient-centered home care, with emphasis on assessments, care plan revisions, and HIPAA compliance.

The ideal candidate holds an active Massachusetts RN license, BLS, and 1+ year of direct care experience, with preferred exposure to OASIS/EPIC and home care settings. Influenza vaccination is required.

Qualifications

• Active Massachusetts RN license.
• BLS Certification (American Heart Association).
• Associate's Degree or BSN from an accredited nursing program (BSN preferred).
• Minimum 1 year of direct patient care nursing experience.
• Public health or home care nursing experience preferred.
• OASIS and/or EPIC experience preferred.
• Influenza vaccination required as a condition of employment.

Responsibilities

• Plan, organize, and direct home health services.
• Build services from community resources to meet patient and family needs in homes and communities.
• Complete and document initial patient and family assessments.

• Administer physician-prescribed medications and treatments.
• Instruct, supervise, and evaluate home health aide care every two weeks.

• Plan, organize, and direct home care services


• Build services from community resources to meet patient and family needs in their homes and communities


• Complete and document initial patient and family assessments


• Initiate and revise plans of care as patient status and needs change


• Administer physician-prescribed medications and treatments


• Instruct, supervise, and evaluate home health aide care every two weeks


• Identify discharge planning needs


• Maintain confidentiality under agency and Federal HIPAA guidelines


• Complete OASIS documentation


• Complete timely documentation through EPIC

Requirements

• Active valid Massachusetts RN license


• Active American Health Association BLS certification


• Associate's Degree or BSN from an accredited school of nursing (BSN preferred)


• At least 1 year of direct care nursing experience


• Public health or home care nursing experience preferred


• OASIS and/or EPIC experience preferred


• Must be vaccinated against influenza as a condition of employment
