Job summary

Jobtailor is seeking a Registered Nurse to lead Home Health operations in Fort Worth, TX. You will oversee patient care services, coordinate care across families, physicians, and facilities, and ensure timely starts of care with regulatory compliance.

Job description

• Oversee patient care services, staff assignments, referrals, and care coordination
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities
• Ensure timely start of care and ongoing compliance with applicable regulatory requirements
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness
• Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care
• Provide guidance and coaching to direct care team members
• Assist with interviewing and hiring clinical staff
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations
• Perform field work as needed

Requirements

• Associate degree in a Nursing related field required
• 2 years home care experience as a registered nurse in a Home Health or Hospice environment
• 1 year of supervisory experience preferred
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase and Workday
• Knowledge of Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
