Job summary

Trilogy Health Services in Kalida, OH is seeking a Registered Nurse (RN) to provide direct, high-quality clinical care and to serve as Team Leader for patient care staff. You will lead a team, apply the nursing process from admission through discharge, monitor residents daily, and coordinate with the charge nurse, physician, and DON.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, and family teaching are carried out per policy.
• Utilize the nursing process to deliver patient care from admission to discharge and return to family/community.
• Visit residents daily to observe status and notify charge nurse/physician/DON of changes in condition.
• Identify resident problems and emergencies and initiate lifesaving measures in physician absence.
• Perform administrative duties such as completing forms, reports, evaluations, and charting.
