Job summary

Trilogy is hiring a Registered Nurse (RN) to lead a direct care team and deliver high-quality clinical care. You will supervise medications, treatments, documentation, family teaching, and care planning in a long-term care setting, ensuring smooth transitions from admission to discharge.

Expect to observe residents daily, assess changes, and coordinate with the DON, physicians, and charge nurse. The role includes weekend shifts with a special Weekend Package incentive and potential differential

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferences.
• Deliver nursing care using the nursing process from admission through discharge and return to family/community.
• Visit residents daily to observe status and notify charge nurse/physician/DON of changes.
• Identify resident problems and emergency situations and initiate lifesaving measures when needed.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
• Weekend schedule includes 2 (8h) or 12h shifts with a Weekend Package incentive.

WHAT WE'RE LOOKING FOR:

The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.

Key Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit your residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing (DON) when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
• The Weekend Package Program details: Works a minimum of 2*(8hr) or (12hr) shifts every weekend The Weekend Package incentive is an additional $7.00/hour working between the hours of 6pm on Friday to 7am on Monday. Additional shift differentials apply when working evening or night shifts on the weekends.
