Job summary

North Houston Transitional Care in Houston, TX is seeking an LPN/LVN supervisor to provide direct nursing care and oversee nursing assistants. The role ensures compliance with federal, state, and local standards and coordinates daily operations.

You will lead shift planning, ensure nursing staff training, review care plans, document accurately, admit/transfer/discharge residents, and maintain the Nursing Service Procedures Manual. Strong leadership and communication are essential.

Qualifications

Must possess a Nursing Degree and a valid Texas LPN/LVN license.
English language proficiency in reading, writing and speaking.
Ability to read technical procedures and policy manuals.
Strong leadership and supervisory abilities are required.
Experience coordinating nursing services in a long-term care setting.

Responsibilities

Direct the day-to-day nursing functions and supervise nursing assistants.
Ensure compliance with federal, state and local regulations and facility policies.
Plan shifts, services, and activities for residents.
Maintain the Nursing Service Procedures Manual and ensure staff adherence.
Complete and review resident care plans, charting and documentation.
Coordinate admissions, transfers and discharges and communicate with families.
