Job summary

Houston Transitional Care seeks a capable LPN/LVN to provide direct nursing care to residents and supervise day-to-day nursing activities performed by nursing assistants.

You will ensure compliance with federal, state, and local standards, supervise staff, and contribute to the facility's quality of care through proper documentation, care planning, and liaison with physicians and管理 team.

Qualifications

• Must possess nursing degree from an accredited program (LPN/LVN).
• Current, active license to practice as an LPN/LVN in the state.

Responsibilities

• Direct the day-to-day functions of the nursing assistants in accordance with current rules and guidelines.
• Ensure nursing staff compliance with facility policies and procedures.
• Review and revise department policies and procedures as needed.
• Plan shifts, services, programs, and activities with nursing staff and support personnel.
• Maintain and update Nursing Service Procedures Manual and ensure adherence.
• Document and report shift activities and outcomes to leadership.
• Coordinate with other resident services to maintain overall resident care.
• Provide leadership during rounds and ensure CNAs are enrolled in approved training.
• Observe and enforce safety, sanitation, and infection control standards.
• Oversee care planning, charting, and documentation for residents.
