Job summary

North Houston Transitional Care in Houston, TX seeks an experienced LPN/LVN for direct nursing care and supervision of nursing assistants in a long-term care facility. You will ensure adherence to current federal, state, and local standards while coordinating day-to-day nursing services.

Responsibilities include charting, medication administration, staff supervision, training, and maintaining resident safety and quality care.

Qualifications

  • Must possess nursing degree or be graduate of an approved LPN/LVN program.
  • Must possess current, active license to practice as an LPN/LVN in the state.
  • Must be able to read, write, speak, and understand English.
  • Current CPR certification.

Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the facility.
  • Ensure that all nursing personnel comply with written policies and procedures.
  • Participate in developing, maintaining, and implementing facility quality assurance programs.
  • Maintain confidentiality of resident records in accordance with regulations.
  • Provide leadership to nursing personnel assigned to the unit/shift.
  • Meet with shift nursing personnel to identify and correct problem areas and improve services.
  • Ensure nursing service personnel compliance with job descriptions.
