Job summary

Houston Transitional Care in is seeking an LPN/LVN supervisor to provide direct nursing care oversight and to lead the nursing staff in a long-term care setting. You will ensure adherence to policies, review care plans, and coordinate with physicians as residents’ needs evolve.

The role requires strong clinical knowledge, leadership, and the ability to manage day-to-day nursing operations, including documentation and QA activities.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Must possess a current, unencumbered LPN/LVN license in this state.
• Must be able to read, write, and understand English.
• Strong leadership and supervisory abilities are required.

Responsibilities

• Direct day-to-day nursing assistant activities and supervise staff.
• Ensure compliance with facility policies and applicable regulations.
• Maintain nursing service procedures and discharge planning.
• Complete and review care plans, orders, and documentation.
• Coordinate with physicians and respond to changes in resident condition.
