Job summary

HoustonTransitionalCare is seeking an LPN/LVN to oversee nursing staff and ensure the delivery of high-quality patient care in a long-term care setting. The role requires leading day-to-day nursing operations, supervising CNAs, and ensuring adherence to policies and procedures.

The candidate will participate in planning and coordinating patient care, discharge planning, and collaboration with physicians and other health professionals. A valid Texas license and CPR certification are essential.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN / LVN in this state.
• Must be able to read, write, speak, and understand English.
• Current CPR certification.

Responsibilities

• Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the facility.
• Lead staffing planning, supervise nursing staff, and ensure compliance with policies and procedures.
• Review and maintain Nursing Service Procedures Manual and shift reports.
