Job summary

Moments Hospice is seeking an RN Clinical Manager/Team Director to orchestrate holistic, patient-centered care with an interdisciplinary team. You will lead nurses, aides, social workers, chaplains, and volunteers while ensuring regulatory compliance and high-quality outcomes.

You'll drive care coordination with families and physicians, develop policies, manage staffing, and participate in after-hours coverage to support seamless hospice services.

Qualifications

• RN license and minimum 3 years healthcare experience (prefer hospice).
• Bachelor's degree strongly preferred.
• Leadership or management experience a major plus.
• Valid driver's license required.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews; foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
