Job summary

Moments Hospice is seeking an experienced Hospice RN Clinical Manager/Team Director to lead an interdisciplinary team and coordinate end-of-life care for patients and families in Rochester, MN. The role focuses on clinical leadership, policy development, and seamless caregiver communication.

You will oversee nurses, aides, social workers, chaplains, and volunteers, ensuring regulatory compliance and high-quality care while supporting a positive work environment and ongoing staff development.

Qualifications

• RN license with a minimum of 3 years healthcare experience, preferably hospice or related settings.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.
• Valid driver's license is required.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as central hub for communication with families, physicians and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
