Job summary

Moments Hospice is seeking a skilled RN to serve as Clinical Manager/Team Director, leading an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers. You will ensure holistic, compliant hospice care and coordinate with families, physicians, and providers.

In this leadership role, you will drive care planning, policy updates, and performance reviews while participating in after-hours coverage and on-call duties to maintain seamless patient care.

Qualifications

• RN license required with healthcare experience in hospice or related settings.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.
• Valid driver's license required.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
