Job summary

Moments Hospice seeks an experienced Hospice RN Clinical Manager/Team Director to lead an interdisciplinary team and coordinate patient care across families, physicians, and providers. The role emphasizes compassionate leadership, regulatory compliance, and continuous quality improvement.

Travel is a core aspect, with a company car provided. A strong clinical background in hospice and proven leadership are essential for guiding staff and ensuring exemplary patient outcomes in our Minnesota

Qualifications

• RN license required and active.
• Bachelor's degree strongly preferred.
• Leadership/management experience highly valued.
• Valid driver's license required.

Responsibilities

• Lead an interdisciplinary team of clinicians and support staff.
• Ensure compliance with hospice regulations and standards.
• Coordinate care plans and family communication.
• Lead meetings and document patient needs and plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews.
• Act as central communication hub with families and providers.
• Monitor quality metrics and caseload management.
• Participate in after-hours and on-call duties.
• Promote a collaborative work culture and optimal resource use.

Qualifications:

• Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings.
• Bachelor's degree strongly preferred.
• Leadership or management experience a major plus.
• Valid driver's license (required)
