Job summary

Moments Hospice in Minneapolis, MN is seeking a Hospice RN Clinical Manager/Team Director to lead an interdisciplinary care team and coordinate services for patients and families at end-of-life journeys.

You will oversee nurses, aides, social workers, chaplains, and other clinicians, ensure regulatory compliance, facilitate communication, and drive policy development while participating in after-hours on-call coverage. This role offers strong leadership opportunities and comprehensive benefits.

Qualifications

• RN license required; healthcare background relevant to hospice settings preferred.
• Bachelor's degree strongly preferred for leadership roles.
• Leadership or management experience is a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.

• Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings.
• Bachelor's degree strongly preferred.
• Leadership or management experience a major plus.
