Job summary

Moments Hospice seeks an experienced Hospice RN Clinical Manager/Team Director in Wisconsin to lead an interdisciplinary team and coordinate patient care across the organization. You will work with nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers to ensure compassionate, compliant, and holistic end-of-life care.

This role emphasizes communication with families and physicians, ongoing policy development, staff scheduling, on-call duties, and driving quality

Qualifications

• RN license with minimum 3 years healthcare experience, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.
• Valid driver's license required.

Responsibilities

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote collaboration.
