Job summary

Apex Home Health, Hospice & Home Care in Colorado's mountain community seeks a Registered Nurse (RN) to join our compassionate care team, delivering high-quality nursing care to residents and supporting their physical, mental, and psychosocial well-being.

You will conduct ongoing assessments, update care plans, supervise CNAs/LPNs, and collaborate with an interdisciplinary team to uphold CMS 5-star standards and ongoing quality growth.

Qualifications

• Current active RN license in applicable state.
• Associate’s or bachelor’s degree in nursing.
• Preferably with at least one year of skilled nursing experience.
• CPR certification (must be maintained current).
• Strong communication, reasoning, and customer service skills.
• Ability to interpret policies and demonstrate positive attitude towards residents.

Responsibilities

• Delivering high-quality nursing care to residents, ensuring physical, mental, and psychosocial well-being.
• Conducting ongoing assessments and updating care plans based on patient needs.
