Job summary

Home Care Network, Inc. is seeking a skilled RN Supervisor / RN Case Manager to serve Southwest Ohio in a PRN or Part-Time capacity. This field role requires travel to multiple counties with no overnight shifts, offering flexible scheduling and mileage reimbursement.

You will coordinate care, supervise nursing staff, and ensure documentation meets state and agency guidelines, while maintaining high-quality patient outcomes and compliance.

Qualifications

• Graduate of an accredited School of Nursing.
• Minimum 1 year nursing supervisory experience.
• Active RN license in Ohio.

Responsibilities

• Direct, plan, and initiate actions in home care situations.
• Supervise a team of RNs, LPNs, HHAs, and CNAs.
• Document and coordinate patient status changes with care team.
• Ensure safety and confidentiality in all care and documentation.
