Job summary

Home Care Network is seeking a skilled RN Case Manager to join our home health team on a PRN basis, serving Jackson, Vinton, Gallia counties in SE Ohio. The role offers flexible hours, mileage reimbursement, and potential growth to part-time or salaried positions.

You will perform assessments, coordinate care, supervise a multidisciplinary team, and ensure timely communication with physicians and family members while maintaining high quality and HIPAA compliance.

Qualifications

• 1+ year of RN case management experience.
• Experience in home health care and Oasis assessments is a plus.
• Active Ohio RN license.
• CPR and First Aid certifications required.

Responsibilities

• Perform client assessments and coordinate patient care in home settings.
• Direct, plan, and initiate actions; supervise a team of RN, LPN, HHA, and PCA.
• Document patient status and changes; communicate with care team.
• Travel within surrounding counties; mileage is reimbursed.
• Ensure safety and compliance with HIPAA and quality standards.
