Job summary

CareBuilders At Home, Inc. is seeking an Agency RN Supervisor to oversee home care staff and ensure compliant care in North Carolina. The role includes directing nursing teams, performing assessments, and ensuring the plan of care aligns with the NC Nurse Practice Act.

Job description

The agency RN Supervisor shall have the authority and responsibility for direction and supervision of the agency staff and shall meet one or more of the following qualifications:

Reporting

Reports to the President/Owner or designee.

Education

Completion of an accredited Registered Nurse program of study.

Licensure

Current and unencumbered license in the State of North Carolina.

Experience

At least one year of experience as a home care RN supervisor.

Credentials
  • 1. RN license in North Carolina, active and current as verified by primary source verification.
  • 2. Background screening as required by state law.
  • 3. OIG, SAM, OFAC and NSO screening.
  • 4. Current health provider CPR from the American Heart Association at the BLS level or equivalent.
  • 5. Hepatitis B Statement or Declination [OSHA 29 CFR, 1910.1030(f)(2)].
  • 6. TB screening as required by state law.
Environmental Working Conditions
  • 1. Work sites vary, home settings and facilities, such as assisted living.
  • 2. Possible exposure to blood, bodily fluids, and other potentially infectious materials.
  • 3. The physical demands are representative of those that may be required by an employee to successfullyperform the representative duties and responsibilities of the job. Applicant should be able to read medical equipment and documents. Pushing carts and medical equipment may be a component of the job.
  • 4. The following may be required for patient care:
    • Able to perform CPR;
    • lift at least 30 pounds;
    • walk up and down the stairs;
    • position patients;
    • walk; and
    • be able to see.
Representative Duties and Responsibilities
  1. Complies with CareBuilders at Home (CBaH) policies and procedures.
  2. Supervises each type of home care service contained in Section .1100 that is provided by the agency either directly or by contract. Supervises the delivery of services provided by the listed Nurse Aide in accordance with the NC Nurse Practice Act and G.S. 90-171.30.
  3. Verifies core competencies of in-home aides performing personal care skills:
    1. a. Help with prescribed exercises which the client and in-home aides have been taught by a health care practitioner licensed pursuant to G.S. 90;
    2. b. provide or assist with personal care (i.e., bathing, care of mouth, skin and hair);
    3. c. assist with ambulation;
    4. d. assist client with self-administration of medications which are ordered by a physician or other person authorized by state law to prescribe;
    5. e. perform incidental household services which are essential to the client’s care at home; and
    6. f. record and report changes in the client’s condition, family’s situation or needs to an appropriate health care practitioner.
  4. 4. Determines caregiver competency by instruction and return demonstration of tasks.
  5. 5. Makes assessment visits to develop the plan of care.
  6. 6. Signs and dates the plan of care.
  7. 7. Provides a copy of the plan of care to the client or their responsible party.
  8. 8. Ensures the plan of care shall contain the level of assistance required by the client for each ADL.
  9. 9. Conducts the assessment in the client's home to include the client's functional status in the areas of social, mental, physical health, environmental, economic, ADLs, and IADLs.
  10. 10. Available for supervision and consultation while caregiving staff is on duty.
  11. 11. If the RN supervisor delegates duties to in-home caregivers, he or she will remain accountable for the delegated duties.
  12. 12. Available for supervision and consultation while caregiving staff is on duty. Supervision of Hands-on Services and Timeframes: Supervise an in-home caregiver by making a supervisory visit to each client's place of residence every 90 days with or without the in-home caregiver's presence, and annually, while the in-home caregiver is providing care to each client. The supervisory visit shall include review of the client's general condition, progress, and response to the services provided by the in-home caregiver.
    • a. May simultaneously conduct the case review every 90 days as required in Rule .1202.
  13. 14. Supervision Non-hands on Services and Timeframes: In-home companion, sitter or respite services only.
    • a. The agency shall designate an individual to serve as agency administrator for supervision of this category of services.
    • b. The agency administrator shall have the authority and responsibility for administrative direction of the agency.
    • c. The agency administrator shall be a high school graduate, or be certified under the G.E.D. Program,and shall meet one or more of the following qualifications:
    • i. Shall be a health care practitioner as defined in G.S. 90-640(a); or
    • ii. Shall have one (1) year experience in home care, companion, sitter, or respite services, or any other provider licensed pursuant to G.S. 131E or G.S. 122C.10A NCAC 13J.1503(1).
    • d. The agency designated supervisor shall supervise the companion and sitter staff by contacting the client receiving care every three (3) months and by making a supervisory visit to each client’s place of residence at least every six (6) months, with or without the companion or sitter’s presence, and at least annually, while the companion or sitter is in the home providing services to the client.
