Job summary

Heart to Heart Hospice in Houston seeks a Staff Nurse - RN to provide compassionate care. You will observe, document, and report resident conditions, implement the care plan, and communicate changes to the team.

Requirements include a current Texas RN license and a valid driver’s license. Two years RN experience preferred. We offer competitive pay and benefits including health/dental/vision insurance, PTO, holidays, 401(k) with matching, and mileage reimbursement.

Qualifications

• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Observe, assess, document and report resident conditions during rounds.
• Implement the individualized Plan Of Care as directed.
• Identify significant changes in patient condition and take action per policy.
• Oversee and monitor Certified Nurse Aides.
• Discharge residents per facility.
• Complete Nursing Assessments for Physician Calls prior to contacting a physician on change in condition.
• Communicate with the resident's Point of Contact about status or changes.
• Provide training to staff as needed.
• Ensure timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws and Medicare regulations.

What You Will Do:

• During rounds observes, assess, documents and reports resident conditions and/or changes
• Implement the individualized Plan Of Care as directed
• Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures

• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
