Job summary

Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care for individuals with life-limiting illnesses. You will assess needs, coordinate care, and collaborate with families and the IDG to support quality of life.

Requirements include RN license in the state, graduation from an accredited nursing program, a valid driver's license, reliable transportation, and at least two years of clinical experience.

Qualifications

• RN license required
• Two years of RN clinical experience preferred
• Graduate of accredited nursing program (Diploma/ADN/BSN) required

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs
• Implement individualized care plan and revise as needed
• Collaborate with physicians, caregivers, and IDG to coordinate care
• Attend IDG meetings and participate in care planning
• Be on-call as assigned and ensure documentation is timely
• Comply with laws, regulations, and policies

Education

Two years of RN experience preferred in healthcare/clinical setting

Graduate of an accredited nursing program (Diploma/Associate/Bachelor)

Current State Registered Nurse License

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
