Job summary

Heart to Heart Hospice in San Antonio, TX is seeking an RN to join our team as Hospice House Staff Nurse (PRN/Full Time benefits). The ideal candidate will hold an active Texas RN license, have reliable transportation, and bring at least 2 years of RN experience in a healthcare setting.

You will participate in patient rounds, implement care plans, and collaborate with the care team to support residents and families.

Qualifications

• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• During rounds observes, assess, documents and reports resident conditions and/or changes.
• Implement the individualized Plan Of Care as directed.
• Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures.
• Oversees and monitors Certified Nurse Aides.
• Discharge residents per facility.
• Completion of Nursing Assessment for Physician Calls prior to calling Physician on any change in resident condition.
• Communicate with resident’s Point of Contact when they ask for status updates or if there is a change in condition.
• Provide training to other staff as needed.
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
