Heart to Heart Hospice is seeking an RN Case Manager in a full-time role to assess and plan care for patients with life-limiting illnesses. You will coordinate with families and a multidisciplinary team to provide compassionate, comprehensive care in the community.

The ideal candidate will hold a nursing degree, current RN licensure, and a valid driver’s license with reliable transportation. Experience in a healthcare setting is preferred to support timely, compliant patient care delivery.

Formación

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsabilidades

• Initial and ongoing comprehensive assessments of patients and their needs, including ADLs and psychosocial aspects.
• Implement the individualized plan of care and revise as necessary.
• Collaborates with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with the team.
• Perform on-call responsibilities and provide on-call service as assigned.
• Timely submission of documentation and records.
• Ensure compliance with laws, regulations and agency policies.

What You Must Have:

• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
