Job summary

Heart to Heart Hospice in Fort Worth, TX, is seeking a Staff Nurse (RN) for full-time hours to deliver Compassionate Care as part of our care team.

We offer a $2,500 sign-on bonus, competitive pay, and benefits including medical, dental, vision, PTO, and 401(k) with an employer match. You will round, assess, document, and manage patient plans while supervising CNAs and coordinating withCaregivers and families.

Qualifications

• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and active auto liability insurance.
• Two years of RN experience preferred in a healthcare setting.

Responsibilities

• During rounds observes, assess, documents and reports resident conditions and changes.
• Implement the individualized Plan Of Care as directed.
• Identify significant changes in resident condition and take action per policy.
• Oversees and monitors Certified Nurse Aides.
• Discharge residents per facility.
• Complete Nursing Assessments for Physician Calls before contacting the Physician.
• Communicate with resident’s Point of Contact for status updates or changes in condition.
• Provide training to other staff as needed.
• Utilize excellent customer care with internal and external customers.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws and Medicare regulations.

Hospice House Staff Nurse - RN

What You Must Have:

• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• During rounds observes, assess, documents and reports resident conditions and/or changes
