Resources for Human Development (RHD) in Delaware is hiring a full-time Registered Nurse (RN) Staff Nurse for its Delaware Intensive Case Management program. The role focuses on community-based nursing, medication management, and coordinating comprehensive care for individuals with serious mental illness and co-occurring conditions.

You will work with an interdisciplinary team to deliver health education, assessments, and care coordination, with a $70,000 annual salary and a $2,000 sign-on

Qualifications

• Current RN license in Delaware.
• Graduate of an accredited School of Nursing.
• BSN preferred for advancement.
• Requirements include pre-employment TB screen and CPR.
• Ability to obtain/maintain required security clearance.
• Current driver’s license and access to a vehicle.
• Willingness to travel throughout the community.

Responsibilities

• Provide nursing and clinical care to individuals with mental illness.
• Manage medications and treatments and coordinate with the treatment team.
• Perform psychiatric and physical health assessments in community settings.
• Educate participants and families on health and medications.
• Collaborate with physicians, pharmacies, and community resources.
• Participate in on-call responsibilities and crisis intervention.
